Assuming a functional system:
The IS should be viewed as a cross-functional system (working for all parts of the company, can be used by multiple departments) rather than just functional by the end user and his/her department.

Collecting Requirements from Each End User instead of All End Users:
The problem here is that enormous amounts of time could be spent interviewing each individual end user and it could also affect their answers for the analyst.

Asking the Wrong Questions:
The end user tries to answer every question that the analyst asks, but if the analyst asks a wrong question, the end user will answer it probably with a wrong answer. Questions asked must be about the IS environment rather than what the end user needs in order to diagnose a problem.
