Digest and Basic authentication partially working.
Updated doRequest(non file version).
Still a bug sometimes when using basic or digest authentication with
the CupsPrinter or CupsJob class - still tracking it down.
This update is to commit the updated request handling as my hard
drive is sounding a bit funny.
1. Open: Surveys designated "Open" are surveys that are open to the public. Anyone may complete the survey. Users (new and existing) are not required to sign-up or log-in to take or view these surveys.

2. Sign-up: Surveys designated "Sign-up" are surveys that Users must register for before completing the survey. Users must provide an email address in order to log-in to the survey.
