ethical issue between two countries

Background
The medical culture and the health care system are significantly different between the U.S and Taiwan. The case I am going to present is a live experience happened to my brother and, as a native resident growing up in a doctor family in Taiwan for 25 years, I think I can reasonably assume that such case is not uncommon in Taiwan. However, I would also argue that such case maybe as common here in United States as it is in Taiwan.
Case
My brother is a cardiac surgeon in the most prestigious hospital in Taiwan. In my perspective, my brother is a very ethical person because my father, who happened to be a doctor too, always educated him to be a doctor of ethics. In my brother first few years in that hospital, he experienced many surgeries and become more and more competent, in other words, skilled and famous. So, more and more patients come by because of his fame. This fame brought him some unnecessary trouble.
